Process and Impact Evaluation for a Collective Impact Initiative

End Hep C SF

End Hep C SF is a collaboration between SFDPH and several community-based organizations that supports San Franciscans living with and at risk for hepatitis C to maximize their health and wellness. They achieve this through prevention, education, testing, and treatment linkage to reduce morbidity, mortality, and stigma related to hepatitis C. After designing a new strategic plan in 2019, End Hep C SF contracted with Facente Consulting to conduct ongoing evaluation of the project.

Each year, Facente Consulting designs and conducts a process evaluation of End Hep C SF, using surveys,1:1 interviews, and focus groups to reflect on the strengths and weaknesses of End Hep C SF’s function as a collective impact initiative. Starting in 2019, Facente Consulting also worked with End Hep C SF to create a data collection system and public data dashboard that allows for continuous monitoring and programmatic adjustments, using the Results-Based AccountabilityTM framework. This dashboard not only highlights End Hep C SF’s successes through quantitative data, but also includes documentation of the context surrounding each performance measure, epidemiological trends throughout the city, and quality improvement plans.
