The San Francisco Community Health Center (SFCHC) was awarded a five-year grant by the federal Substance Abuse and Mental Health Services Administration (SAMHSA) to support HIV, hepatitis C (HCV), and substance use prevention among unhoused, LGBTQ+ young adults ages 18-24 in San Francisco. SFCHC reached out to Facente Consulting to assist with continuous quality improvement efforts and evaluation of progress towards this project’s objectives. In particular, SFCHC wanted support around administering, analyzing, and reporting the Government Performance and Results Act/National Outcome Measures (GPRA/NOMs) survey required by SAMHSA, and additional evaluation activities tailored to the specific needs of the project.
Throughout this five-year project Facente Consulting supported ongoing evaluation design and implementation of SFCHC’s scope of work by conducting a client needs assessment, designing data collection tools, collecting and analyzing data through focus groups and surveys, participating in ongoing check-ins with the Project Director to discuss ideas for quality improvement, and reporting findings to SFCHC staff and their funders.
