Alameda County Health Care for the Homeless (ACHCH) serves over 9,000 people experiencing homelessness annually in street and shelter settings, convenes a network of service providers, provides training and mortality data evaluation, and leads emergency response efforts for people experiencing homelessness. ACHCH undergoes a strategic planning process every three years. 

Facente Consulting worked with the Alameda County Healthcare for the Homeless (ACHCH) program to facilitate a strategic planning process to determine priorities for the next three years. The process prioritized hearing from stakeholders to ensure community voice in the process. Facente Consulting established clarity and understanding around the determined priorities and provided a landscape analysis to explore strengths, weaknesses, opportunities and threats, and a strategic plan document.
