  • Check back later for guidelines and templates.

Quarterly snapshot due dates are:

  • You will receive an email with a link that is specific to your grant. The email will come from The Facente Consulting Team (via Airtable), noreply+automations@airtableemail.com. This is NOT spam.
  • If you have more than one grant, you will receive an email for each grant.
  • When you click on the link in the email, the form should already have your agency name and grant number in it. If it does not, please contact us.
  • Everything else is the same – same questions, select three (one from each color group).
  • This is NOT a data report, and you do not need to submit any RBA data. The purpose of the quarterly snapshot is two-fold: 1) it will provide us information that can help with identifying future coffee chat topics and TA needs, and 2) since all snapshots will be shared with all the grantees from this FOA, it is an opportunity to see what others are doing, build connections based on common program efforts and challenges, and learn from each other.
  • Remember that these snapshots are intended to be brief – 3 to 5 sentences at most per question. This should be a low-effort, low-stress activity taking 20 minutes or less. You will be asked to provide answers to no more than 3 questions, which you will choose from the list below.

  • Choose 1 question from this group:
    • What are you learning?
    • What program successes or assets could you share with other grantees
    • How has your program demonstrated resilience?
  • Choose 1 question from this group:
    • Share something surprising or exciting that has happened with your program.
    • How have your collaborations enhanced your impact?
    • How is RBA implementation going?
  • Choose 1 question from this group:
    • What challenges are you experiencing?
    • What support do you need?
